It seems the DOP is a combo of drugs, programing (never specified how), monitored by telepath but no telepath involved in making new personality, and monitored and punishment administrated by medical staff... the Alien Machine in seconds can suck the life from one person to cure another the one being sucked will feel intense agony, the machine can be used in a less destructive manner to heal slowly over time by sharing life force the person sharing is visibly exhausted after wards.
